The Parkinson’s Support Group of Green Valley encourages members to stay physically active by reimbursing registered member applicants for exercise class or personal trainer fees.

1. There is a cap of $300 per year per member for reimbursement.
2. To be eligible to receive reimbursement:
   • The person with Parkinson’s must be a registered member of the Parkinson’s Support Group of Green Valley.
   • To qualify as a registered member, a New Member Registration Form must have been completed and submitted to the Database Manager.
   • An Exercise Reimbursement Request Form must be submitted to the Treasurer/Bookkeeper.
   • Original receipts for services received must be submitted to the Treasurer/Bookkeeper with the Reimbursement Request form within 90 days.  Only original receipts will be accepted.
